ANXIETY/PANIC SCREENING QUESTIONNAIRE

1. During the last 6 months, have you been persistently worrying or anxious about several different things (for example, finances, health, work, family, etc) most of the time and more so than other people would worry?   YES or NO

2. During the past month, on more than one occasion did you experience a sudden, unexplained attack of intense fear, anxiety, or panic accompanied by physical symptoms such as shortness of breath, heart palpitations, feelings of choking, dizziness, loss of control, etc., for no apparent reason.   YES or NO 

DISCLAIMER: This Screening questionnaire has been designed by mental health professionals to identify the symptoms of Panic Disorder or Generalized Anxiety Disorder. The screening questionnaire is not a psychiatric evaluation of a mental illness nor can a diagnosis of a mental illness be made through this screening process. This can only be done by a clinical evaluation by a mental health professional.

Do You Need an Anxiety/Panic Program?

If you feel that your anxiety/panic problem is effecting you, if it is having an impact on your relationships and on important parts of your life, you might consider an Anxiety/Panic Program  to learn how to handle it better. A highly trained licensed psychotherapist can work with you in developing a range of techniques for managing anxiety / panic, and by changing your thinking and your behavior. When you talk to a prospective therapist, tell her or him that you have problems with anxiety/panic, and ask about his or her approach to anxiety.

With a good Anxiety/Panic Program a   person can move to a more effective level in about 12 weeks  depending on the circumstances and the techniques used.
